Sports Physicals at School – Tuesday, May 13
HCC Network will be on campus during the school day Tuesday, May 13 to conduct sports physicals for our student-athletes!
HCC flyer (which includes a sign-up QR code) and the MSHSAA Pre-participation Physical Form can be picked up in the office.
Here's what needs to happen:
Parents/guardians must complete the online form using the QR code on the flyer.
Fill out the first two pages of the physical form before May 13. Click here for the form: https://5il.co/3fb0b
Students must bring the completed packet to school on Tuesday, May 13.
HCC will complete pages 3 & 5 during the physical and return the packet to the student.
Once PRIVIT resets (tentatively June 1), families can upload only page 5 and complete the online signatures for next school year.
